Healthcare Software: Why Compliance Comes Before Features
Healthcare projects are the one category where we push back hardest on skipping ahead to features, because the compliance requirements aren't a checkbox at the end — they shape the architecture from the very first decision, and retrofitting them later is close to a rebuild.
Why this is different from a typical project
Patient data has legal protection requirements that go well beyond the general privacy basics most business websites need — specific rules about who can access data, how it's encrypted at rest and in transit, audit logs of every access, and data handling agreements with every vendor that touches it.
Decisions that need to happen before a single feature is designed
- Where data is hosted, and whether that hosting provider will sign the compliance agreements your project legally requires.
- Who gets access to what, modeled explicitly from day one — not added as an afterthought once real patient data is already flowing through the system.
- Audit logging built into the architecture, not bolted on later, since compliance often requires proving who accessed what and when.
- A clear incident response plan before launch, not written reactively after something goes wrong.
In a typical product, security is one of many priorities. In healthcare software, it's the priority everything else is built around.
This is also why "move fast" doesn't really apply here the way it does in most other custom software work — the compliance foundation has to be solid before the first real feature ships, not improved iteratively afterward.
